Paiute and King WIC Location Changes

If you have been using Paiute or King WIC clinics, services for these clinics are now being provided at our 48^th Street location.
We can also help you find a new clinic that is more convenient for you.

WIC Appointments are Now Available by Phone, Video Chat, or In-person
Even though we are still doing most appointments over the phone or on video chat, in-person appointments are now available by request at most of our clinics.

How to Apply for WIC

Visit a clinic near you to apply (walk-ins are served after previously scheduled appointments).

When you visit, please bring the following items:

A form of identification (for each person applying) —like a photo ID, birth certificate, or immunization record
Proof of address—like a current state ID or utility bill
Proof of income—like the last 30 days of pay stubs or a letter from AHCCCS, SNAP, TANF, or Section 8 housing
Recent height/length, weight, and hemoglobin from medical provider OR you can come to our clinic for your appointment
